Abstract

본 증례는 하악좌측 제 1대구치의 맹출지연을 주소로 내원한 7세 남아로서 임상적, 방사
선학적, 조직 병 리 학적으로 법랑모치아종으로 확진된 예이다. 또한 본 증례는 발육중인 치
낭, 복합치 아종, 복잡치아종 및 낭성치아종과의 감별을 요하는 희유한 예로서 다음과 같은
결과를 얻었다.
1 임상적으로 특이한 증상 및 징후는 없었다.
2. 방사선사진상에서 맹출지연된 치아의 치관주위 병소와 후방의 비정상적으로 확대된 치
낭 및 내부의 불규칙한 방사선불투과상이 관찰되었다. 맹출지연된 하악 좌측 제 1대구치 치
관부에 명료한 경계를 보이는 방사선투과상 내부에 여러개의 작은 방사선불투과상이 관찰되
었다. 후방의 병소는 비정상적으로 확대된 치낭의 방사선투과성 공간내에 불규칙하게 형성
된 방사선불투과상으로 관찰되었으며 주위의 경계는 명료한 원형의 형태를 가지지만 매복된
전방치아 후방부위와 연결된 소견이 관찰되었다.
3. 조직병리학적으로 병소는 얇은 섬유조직으로 둘러싸인 치아경조직을 포함한 조직과 상
부의 상아질과 하부의 법랑질로 구성된 치아종부가 관찰되었다. 또한 치낭 주위에 이장되어
있는 원주형 상피세포내부에 성상세망으로 구성된 전형적인 여포형의 법랑모세포종 소견도
관찰되었다.

Ameloblastic odontoma is a mixed odontogenic tumor, which is characterized as being
extremely rare, displaying aggressive clinical behavior, resembling a mixed radiopaque
and radiolucent radiographic appearance and containing both ameloblasloma and a
composite odontoma. Seven-year-old boy complaining of delayed eruption of the left
lower permanent 1st molar came to the department of Oral & Maxillofacial Radiology at
Chonbuk National University Hospital.
The list of our radiographic differential diagnosis included the dental follicle, compound
odontoma, complex odontoma and Cystic odontoma. The microscopic analysis showed the
lesion with the characteristics of an ameloblastic odontoma.
The obtained results were as follows;
1. The area asymptomatic, but 4he delayed eruption of the left lower permanent 1st
molar was observed.
2. Radiographically, well-declined widened pericoronal radiolucency containing several
tooth-like radiopacities involving the impacted left lower permanent 1st molar and
well-declined round radiolucency containing irregular radiopaque mass were observed.
3. Hislopathologically, several tooth-like structures, odontoma components with the
area of typical enamel, dentin and pulp ameloblastic components with typical follicular
ameloblastoma were observed.
